1: I've been in Web design since 1998 and never heard of "type page," "text page," "trim size" or "open line" used in a Web context (I have 12 years of newspaper experience, and have heard those terms used in print). I'd go with D but I think your instructor made up these terms.
2. Again, these terms are used in printing, not Web work. Books are actually divided most commonly into "front matter" (copyright / ISBN, foreward, preface, perhaps reviews, etc.), chapters and "back matter" (afterward, acknowledgments, author bio, appendices, index, etc.) I'd go with D but again, your instructor made this up.
3. C: The intended audience. You must make technical concessions to the printer, and the author is often going to want to have some say in presentation, but as a rule, you design for the target audience. Collateral you make for teens generally will not look like similar information you make for seniors.
I wish there was just one manual book for that but there isn't. There are several sites and resources online. One funny one I found at Hastings books is the Web site design and web building for dummies book. You could also try microsofts web builder programs as well. Good luck. Or if you can afford it most of the local community colleges in your area might offer training classes on it.

It depends upon quality and design of webpages. A good designed webpage cost you around US $100 and rest of pages may cost you from $30 to $70. I want some more fucntionality like dynamic pages add $30 to it.

I am taking online classes at Palomar College (http://www.palomar.edu) and they offer everything from basic HTML classes to more advanced stuff like JavaScript/AJAX, PHP, ASP, Java, etc. Some classes have an in class orientation, but I didn't go to any of them and was still able to take the classes, so you should be able to take them from anywhere. If you are just looking to learn for free and not going for a degree, I would listen to the previous suggestion and check out the tutorials at w3schools.com

I don't know for sure if there is a program specifically for Web Development/Programming, but you probably could major in computer programming specializing in Web Development/Programming.
